How about googling "mastectomy". Mastectomy is when the bread is completely removed, it is usually performed on someone with breast cancer. So her nipple isn't there because it was removed due to possible breast damage, breast cancer, etc. Also the comment says she had a saline implant [that way when she wears clothes it looks like she is normal]
